糖尿病并脑梗死患者血小板及可溶性黏附分子含量变化 被引量:1

Changes of the contents of solvable adhesion molecule and platelet in patients with diabetes mellitus complicating with cerebral infarction

摘要 目的探讨糖尿病并脑梗死患者外周血血小板及可溶性黏附分子含量的变化及临床意义。方法采用流式细胞仪和酶联免疫吸附法测定33例糖尿病并脑梗死患者和42例单纯脑梗死患者外周血血小板膜糖蛋白Ⅱb-Ⅲa复合物的a亚单位(CD41)、P-选择素(CD62p)的表达,血清中可溶性P-选择素(sP-选择素)和可溶性细胞间黏附分子1(sICAM-1)的水平。结果与单纯脑梗死患者相比,糖尿病并脑梗死患者血小板CD41、CD62p的表达和血清中sP-选择素和s1CAM-1水平显著升高;Logistic多元回归分析证实sICAM-1和sP-选择素是糖尿病并脑梗死患者的危险因素。结论血小板和内皮细胞的活化在糖尿病并脑梗死患者发病中起重要作用,糖尿病患者脑缺血后更易导致脑损伤。 Objective It is to study the changes and its clinical significance of the contents of solvable adhesion molecule and platelet in peripheral blood in patients with diabetes mellitus (DM) complicating with cerebral infarction (CI). Methods The expression of CD41 and CD62p in the platelet membrane glycoprotein Ⅱ b- Ⅲ a complex and the levels of serttm antin- P selectin antibody and sICAM- 1 in 33 cases of DM complicating with CI and 42 cases of simple CI were determined by flow cytometry and ELIA. Results Combined with patients with simple CI, the expressions of CD41 and CD62p and the levels of serum antin - P selectin antibody and sICAM - 1 obviously increased in patients with DM complicating with CI. Logistic multiple regression analysis proved that sICAM - 1 and antin - P selectin antibody were the dangerous factors of DM complicating with CI. Conclusion The activation of platelet and endothelial cell play an important part during the course of DM complicating with CI. Cerebral ischemia can lead to cerebral injury more easily in patients with DM.
