摘要
目的探讨体外循环前后人肺静脉血中基质金属蛋白酶9(MMP-9)和金属蛋白酶组织抑制因子(TIMP-1)表达及浓度变化。方法选择25例接受心脏手术的患者,其中20例在体外循环下手术,5例未在体外循环下手术。于体外循环(CPB)开始时(进胸时)、CPB结束时(关胸时)在肺静脉采取血标本,用酶联法(ELISA)检测各个标本血浆中MMP-9及TIMP-1的浓度;用逆转录聚合链反应(RT-PCR)测定MMP-9、TIMP-1的表达。结果CPB后人肺静脉血中MMP-9/TIMP-1明显升高,白细胞MMP-9表达及浓度增加,TIMP-1浓度及表达下降,而未经CPB的患者肺静脉血中MMP-9和TIMP-1表达及浓度无明显变化。结论鉴于MMP-9特异性降解作用,推测MMP-9、TIMP-1的变化与CPB后肺损伤的病理变化可能有一定关联。
Objective To elucidate expression and concentration change of matrix metaloproteinase(MMP) -9 and tissue inhabitor of metalloproteinase(TIMP) -1 in human pulmonary vein blood after cardiopulmonary bypass (CPB). Methods Total 25 patients who have performed the heart surgery were choosed in our study,and among them,20 patients were underwent external circulation while 5 patients were not received CPB. Human blood samples were collected from the pulmonary vein at the commence of CPB ( or of surgery) and the termination of CPB ( or of surgery) respectively. Enzyme - linked immunosorbent assay (ELISA) and reverse transcription -polymerase chain reaction ( RT - PCR) was used for detection of MMP -9 and TIMP - 1. Results After CPB,the ratio of MMP - 9/TIMP - 1 increaed;expression and concentration of MMP -9 signicantly increased, and TIMP - 1 in plasma decreased;however, MMP -9 and TIMP - 1 did not change after surgery without undergoing CPB. Conclusion MMP -9 ,TIMP - 1 may be involved in the pathogenesis of lung injury undergoing cardiopulmonary bypass.
