听神经瘤20例手术治疗体会

Surgical intervention therapeutics Experience on 20 Acoustic Neuroma

目的总结听神经瘤手术切除的经验与体会。方法对20例大型听神经瘤实施切除,其中7例经单侧枕下旁正中入路,其余13例为枕下乙状窦后入路。结果肿瘤全切14例,占70%,次全切5例,大部切除1例。全组无手术死亡。面神经解剖保留率为90%(18/20),耳蜗神经解剖保瘤率为25%(5/20)。结论绝大部分大型听神经瘤均采用纯外科手术,治疗效果较好,手术时应力争保留面神经和听神经。

Objective To sum up the surgical experience of the operation on resection the acoustic nerve tumor. Methods Large acoustic nerve tumors in 20 case were reseited of them,7 cases were through the exact center of the unilateral suboccipital pillow path,the other were through the suboccipital sigmoid sinus path. Results 14 of the all acoustic nerve tumor were resected thoroughly （approximately 70 percent） ,5 were subtotal excision （about 25 percent） ,the rest was mostly resected. No patieut died of surgical operation. The rate of the facial nerve reservation in anatomy Was 90 percent （18/20） ,and the cochlear nerve reservation was 25 percent（5/20）. Conclusion Our date showed that the majority Patieuts with acoustic nerve tumor can be cured by the pure surgical operation with better effect. But we should emphasis on the reservation of the facial nerve and acoustic nerve in the surgical operation.
