青春期多囊卵巢综合征的临床治疗

Clinical Treatment of Adolescent Polycystic Ovarian Syndrome

目的观察二甲双胍联合达因-35对青春期多囊卵巢综合征的治疗作用。方法选择60例青少年多囊卵巢综合征患者,年龄15～25岁。随机分为A、B、C三组,A组18例服用二甲双胍;B组15例服用达因-35治疗;C组27例联合服用二甲双胍和达因-35。三组用药时间均为3个月。测定患者血清卵泡刺激素(FSH)、黄体生成素(LH)和睾酮(T),测量腰/臀比值(WHR)及体重指数(BMI)并进行治疗前后的比较。结果A、C两组治疗后LH、LH/FSH、T及WHR均较治疗前明显降低(P＜0．05或P＜0．01)且两组间差异无显著性(P＞0．05);B组治疗后LH、LH/FSH、T较前明显降低(P＜0．01)但WHR明显升高(P＜0．05)。A、B、C 3组治疗后各项指标比较:LH、LH/FSH及T差异无显著性(P＞0．05),而B组WHR明显高于A、C两组(P＜0．05)。结论二甲双胍联合达因-35可以有效治疗青春期多囊卵巢综合征。

Objective To investigate the therapeutic effects of combination of metformin with Diane -35 on cases with adolescent polycystic ovarian syndrome （PCOS）. Methods sixty patients with PCOS, aged 15 -25 years, were randomly divided into 3 groups： metformin group （A = 18）, Diane-35 group （B = 15）, Diane-35 ＋ metform in group（ C =27）. After a baseline work including serum follicle stimulating hormone （FSH）, luterinzing hormone （LH）, testosterone （T）, waist/hipratio （WHR） and body mass inder （BMI）, at the end of 3 months of treatment, all subjects were re -evalua- ted. Results The level of LH, LH/FSH,T and WHR decreased significantly in group A and C after treatment （ P〈0. 05 or P〈0. 01）, but there was no significant difference between the two groups （P〉0. 05）. In group B, significant decrease of LH, LH/FSH and T were observed compared with pretreatment （ P 〈0. 01 ）, while WHR increased markedly （ P 〈0. 05）. There were no significant changes of LH, LH/FSH and T among these three groups（ P〉0. 05）, but the WHR of group B increased significantly compared with the other two groups（ P 〈0.05）. Conclusions Diane -35 combined with metformin is an effective therapy in dealing with adolescent patients with polycystic ovarian syndrome.
