口腔颌面部恶性肿瘤单体素氢质子磁共振波谱的初步研究 被引量:8

Oral-maxillofacial malignant tumors:an initial study with in vivo single-voxel ~1H MR spectroscopy

摘要 目的探讨用单休素氢质子磁共振(~1H MR)波谱诊断口腔颌面部软组织恶性肿瘤的可行性。方法回顾分析42例经~1H MR 检查并经病理证实为软组织恶性肿瘤的病例。~1H MR 波谱空间定位采用点分辨波谱法,同波时间144 ms。以肿瘤内胆碱化合物(化学位移为3.2×10^(-6))的检出为评价标准。结果 42例恶性肿瘤中,检出胆碱化合物者37例,包括15例鳞状细胞癌,9例涎腺癌,13例肉瘤和恶性淋巴瘤。结论单休素~1H MR 波谱能为口腔颌面部软组织恶件肿瘤的诊断提供有益信息。多数恶性肿瘤以含胆碱化合物为特点,其中肉瘤和恶性淋巴瘤内胆碱化合物的检出高于上皮性癌。 Objective To investigate the possibility of using single-voxel ^1H MR spectroscopy to evaluate malignant soft tissue tumors of oral-maxillofacial region. Methods Forty-two patients with oral- maxillofacial malignant tumors, examined by single-voxel ^1H MR spectroscopy at al. 5 T MR unit before trearment and confirmed by histo-pathological examination, were retrospectively studied. The localization for ^1H MR spectroscopy was used with a point-resolved spectroscopy (PRESS) at echo time of 144 ms. Choline (Cho), one of tumor's metabolites, was considered as a criterion to assess the malignant tumors. Results Of 42 oral-maxillofacial malignant tumors, the Cho peak was identified in 37 lesions, including 15 of 17 squamous cell carcinomas, 9 of 12 salivary carcinomas, and 13 of 13 sarcomas and non-Hodgkin lymphomas. Conclusions Single-voxel ^1H MR spectroscopy might provide useful information for the assessment of malignant soft tissue tumors in the oral-maxillofacial region. Most malignant tumors in this study were characteristic of containing Cho metabolites. The Cho peak was more frequently recognized in the sarcomas and non-Hedgkin lymphomas than in the carcinomas.
