时间调整敏感度编码与全局自动校准部分并行采集技术在心功能评价中的比较 被引量:13

Cardiac functional analysis with time-adaptive sensitivity encoding technique:comparison with generalized autocalibrating partially parallel acquisitions technique

摘要 目的对比分析单次屏气时间调整敏感度编码(TSENSE)和多次屏气全局自动校准部分并行采集(GRAPPA)技术结合真实稳态进动(true FISP)电影,在评价左心室功能中的价值。方法对健康组30例健康成年人和疾病组32例心脏器质性病变患者用数字表法随机行单次屏气 TSENSE和同层数多次屏气 GRAPPA 左心室短轴电影进行心功能检查。所得图像经心功能分析软件分析后分别获得个体两种方法的左心室射血分数(EF)值、左心室舒张末容积(EDV)、左心室收缩末容积(ESV)、心搏量(SV)、心输出量(CO)、心肌平均重量(MM)和心率(HR),采用配对 t 检验法对两组两种方法所得各心功能指标进行比较。结果健康组 TSENSE CO 较 GRAPPA 组低[分别为(3.8±0.8)、(4.0±0.9)L/min],差异有统计学意义(t=2.262,P<0.05);其余心功能参数两种方法差异均无统计学意义(P>0.05)。疾病组左心室功能各指标,包括 EF 值、EDV、ESV、SV、CO 和 MM 两种方法差异均无统计学意义(P>0.05)。结论单次屏气 TSENSE true FISP 电影分析左心室功能简便、快速、准确,基本可以替代多次屏气 GRAPPA 电影,具有重要的临床实用价值。 Objective To assess the clinical application value of time-adaptive sensitivity encoding (TSENSE) with single breath hold true fast imaging with steady-state precession (true FISP) cine sequences for the left ventricular function in comparison with generalized autocalibrating partially parallel acquisitions (GRAPPA) multi-breath hold true FISP cine sequences. Methods Thirty healthy adult volunteers [ male 21, female 9, age (48 ± 16) y ] and 32 with cardiac disease [ male 28, female 4, age (49 ± 15 ) y ] were studied for evaluating left ventricular function with use of multi-breath hold GRAPPA and single breath hold TSENSE true FISP left ventricular short axis cine. The ventricular function parameters including ejection fraction (EF), end-diastolic volume (EDV) end-systolic volume ( ESV), stroke volume (SV) , cardiac output (CO) , mean myocardial mass (MM) and heart rate (HR) were calculated by using analysis software ( Argus, Simens) after drawing the contour of endocardium and epicardium on TSENSE and GRAPPA cines. A paired-samples t test was used to analyze the difference. Results In volunteers group, TSENSE technique leads to a decrease in CO comparing with GRAPPA technique [ ( 3. 8 ± 0. 8 ) L/min, (4.0 ± 0.9) L/min;t =2.262, P〈0.05]. There was no significant difference for EF, EDV, ESV, SV and MM ( P 〉 0. 05). In patients group, there was no significant difference for all ventricular functional parameters including EF, EDV, ESV, SV, CO and MM ( P 〉 0. 05 ). Conclusion Compared with multi-breath hold GRAPPA technique, single breath hold TSENSE technique is a fast, accurate and reproducible method and can be an substitute for GRAPPA to evaluate ventricular function.
