严格审查中药处方与临床合理用药

Examination of Prescription of Traditional Chinese Medicine and Rational Clinical Drug Use

目的减少或降低药物的有害作用,确保患者用药安全、有效、经济。方法审查中药处方32086张,询问患者用药情况,参照诊断结果及病历对发现的问题给予纠正,并向医师提出合理化建议。结果发现问题1200例,占审方总数的3.7%,其中因剂量、用法、合并用药产生的问题分别为290例、114例、792例,分别占问题总数的24.5%、9.5%、66%。结论药师审查处方参与药物治疗,对患者合理用药起到了积极的促进作用。

OBJECTIVE： To reduce or alleviate the adverse reaction of drugs for the safety, efficacy and economy of patients in drug use. METHODS： 32 086 traditional Chinese medicine prescriptions sheets were examined and the drug administration of patients was inquired. Suggestions for rationalization and rectification were given to the physicians with reference to the diagnosis and case history.RESULTS： There were 1 200 cases of improper treatment, which was 3.7% of the total pre scriptions including 290 cases of dosage problem, 114 cases of application problem and 792 cases of complication promblem which were 24.5% ,9.5% ,66% of the total prescriptions respectively. CONCLUTIONS：Pharmacist＇s examination of prescriptions as a way to participate in drug therapy can positively contribute to rational use of drugs in patients.
