老年冠状动脉介入术并发低血压的观察及护理

目的 分析老年冠状动脉介入术并发低血压的原因及护理策略。方法 通过对296例老年冠状动脉介入术并发的低血压进行回顾性分析。结果 在296例冠状动脉介入术患者中并发低血压6例．占2．0％，其中1例因对造影剂过敏．发生在术中．且及时得到救治．4例发生在拔出鞘管后10min内．1例发生在术后3h内。经及时处理．在30～60min内生命体征恢复正常。结论 护士了解老年冠心病病情重、症状不典型、多种疾病共存、体质差、记忆力理解力及配合力差、易出现并发症的特点；了解冠脉介入术适应症、禁忌症及常见并发症；熟悉并发低血压的常见原因；对患者术前术后的心理护理；密切观察患者在术中术后心率、血压、心电图、神志、尿量的变化；迅速判断低血压发生的原因并配合医生作相应处理是预防和救治并发低血压的重要保证。

Objective：To analyse the reasons and nursing strategies of hypotension after the coronary artery intervention operation for old age. Mehtods：Take corresponding nursing measurs with clinical nursing observation to aim at the reasons for hypotension. Results： Hypotension occured in 6 of 296 cases who accepted the coronary artery intervention（2.0 %）, 1 case happened in the operation for the hypersensitivity to constrast medium but cured in time,4 cases happened ten minutes after pulling out the sheath and another case happened within 3 hours after the operation. By treating in time,life signs of all the cases recovered normally within 30~60 minutes. Conclusions：The important conditions to prevent and cure hypotension of the patients who accepted the coronary artery intervention operation successfully are as follows：grasp the feature of coronary artery disease for old age; master the indication, contraindication, complication of coronary artery intervention operation and reasons for hypotension; carry out mental nursing; nurse and observe the patients carefully;treat with everything rapidly and promptly.
