偏瘫患者强化上肢训练的价值及简易手功能评定箱的可信性研究

The value of enhanced exercises in stroke patients and the reliability of simple hand function evaluation

本研究目的是验证脑血管意外（ＣＶＡ）患者发病３个月后手功能是否继续恢复及一种简易手功能评定箱（ＳＨＥＦ）的可信性。４６例ＣＶＡ患者，平均病程１１６．７±２３．５天，同时用Ｆｕｇｌ－Ｍｅｙｅｒ法和ＳＨＥＦ法评定，采用强化上肢训练法治疗２５．４±１８．９次后，观察组（３２例）有非常显著的改善（Ｐ＜０．００１），而对照组不明显，两组比较差异也非常显著（Ｐ＜０．００１）。两种评定方法呈显著相关（ｒ＝０．９６２，Ｐ＜０．００１），得出回归方程为Ｙ＝０．６６６Ｘ－０．０５。作者认为病程在３个月以上的患者们有康复价值。ＳＨＥＦ法是一种可信度高、定量化好的评定方法。

Forty-six over 3 months poststroke hemiplegic patients were evaluated with Fugl-Meyer method and Simple Hand Evaluation Facilities(SHEF).The observed group patients(n=32) were treated with enhanced exercises training of the hemiplegic upper-extremity,The evaluation score of the observed group after treatment was significantly higher than before the treatment and the control group(P<0.001).There was a high reliability between the two methods(coefficient of regression,r=0.962,P<0.001).Equation of linear regression was Y=0.666X-0.05.The author came to the conclusion that the patients might get benifit by rehabilitation even after 3 months after stroke.
