摘要
作者报告经单鼻孔蝶窦直接入路切除垂体腺瘤３８例，其中３６例肿瘤近全切除，２例大部切除。术后无并发症。作者认为：此入路与其它经蝶切除鞍区病变的术式相比，对鼻部组织损伤小，不必分离鼻中隔和两侧粘膜，省时，简单，并发症少，值得临床推广。
casesofpituitaryadenomaswereresectedviadirecttransnasaltrans-sphenoidalap-proach.Completeorsubtotaltumorexcisionwasachievedin36patients,andpartialexcisionin2.Theoperativemortalitywas0.Theoperativetechniqueswere,inpatientswithsmalnostrile,a5～6mmsmalincisionwasmadeinthebaseoflateralalar,butthespeculumstraightlyupontotheanteriorwalofthesphenoidalsinus.Asexpandingthespeculum,themiddleseptumwaspressedtotheoppositeside.Undertheoperatingmicroscope,theanteriorwalmucosewasdissectedaway,thebonywaldriloffandthenthesphenoidalsinusapproached.Otherstepswereasusual.Alcasesachievedgoodresult.Theadvantagesofthisapproachwerelessdestructive,lesstime-concuming,lesscomplicationsandeasytoperform.
