摘要
目的比较旋转数字减影血管造影(digitalsubtractionangiography,DSA)与磁共振血管成像(magneticresonanceangiography,MRA)在颅内动脉瘤诊断中的价值。方法40例疑为颅内动脉瘤的患者同时行旋转DSA和MRA检查。评估旋转DSA显示动脉瘤的个数、部位、大小、形态、动脉瘤颈及与载瘤动脉的关系,并与MRA进行比较分析。结果40例患者共40个动脉瘤,旋转DSA检查发现39个,敏感性为97.5%,直径大小为2~30mm。MRA检查发现36个,敏感性为90%,直径大小为4~30mm。旋转DSA检查对动脉瘤的全貌、瘤颈的位置和结构及与周围血管分支的解剖关系均能清晰显示。旋转DSA显示动脉瘤的部位、大小和形态同MRA基本一致。结论旋转DSA技术在颅内动脉瘤诊断和治疗中具有极其重要的作用,在敏感性和特异性方面优于MRA,不但可明确诊断,而且还有利于临床治疗方式的选择。
Objective To compare the value of rotational DSA with MRA in diagnosis of intracranial aneurysms. Methods Forty patients with suspected intracranial aneurysms were performed both rotational DSA and MRA. The findings on rotational DSA and MRA, including the number, site, size, shape,neck and parent vessels of aneurysms, were compared. Results Of 40 patients with intracranial aneurysms,39 were detected by rotational DSA,with a sensitivity of 97.5 % and size of 2 ~ 30 mm. Thirty-six were detected by MRA,with a sensitivity of 90 % and size of 4 ~ 30 mm. Shape of the aneurysms and visibility of the aneurysm necks could be clarified after examination with rotational DSA, and meanwhile, vascular anatomical profile can be indentified with rotational DSA. Rotational DSA was basically the same as MRA in showing the site, size and shape of the aneurysms. Conclusion It is demonstrated that rotational DSA plays an important role in showing the characteristics of the intracranial aneurysms. Rotational DSA is superior to MRA in sensitivity and specificity,it can provide not only definite diagnosis, but also play an important role in selection of therapeutic approach.
