摘要
目的比较血清CysC与其他早期肾损害指标,进而评价其在高血压病肾损害的早期预测作用。方法选取34例在我院健康体检正常者和74例高血压病初诊患者。采用免疫放射比浊法测定血清CysC、尿微量白蛋白、尿α1微球蛋白,酶法测定SCr;分别用单因数方差分析比较正常者与原发性高血压组及不同高血压水平亚组各参数指标的差别;并对各参数指标进行pearson相关性分析。结果各参数指标在高血压组均大于正常组;亚组与正常组比较,血压2级以上,血清CysC(各组数据对数转换后分别为正常对照组:2.92±0.07、高血压病组:2.98±0.14、高血压病1级:2.92±0.07、高血压病2级:2.99±0.10、高血压病3级:3.12±0.17)、尿微量白蛋白、尿α1微球蛋白其值均高于正常对照组;但SCr只有3级高血压才明显高于正常组。相关性分析提示血清CysC与SCr最相关(r=0.8,P<0.001)。结论血清CysC可作为高血压病早期肾损害的评价指标。
Objective To compare Cystatin C (Cys C) with other renal injury markers in patients with primary hypertension. Methods Seventy-four primary hypertensive patients and 34 healthy subjects were included in the study. Serum levels of Cys C, alpha-microglobulin, serum creatinine (SCr), uric albumin were measured in all subjects.The one-way ANOVA test was used to determine the difference between groups.The relationship of serum creatinine and serum levels of Cys C, alpha-microglobulin, uric albumin was assessed using bivariate correlations. Results Serum Cys C(lgCystatinC of every group is: normal:2.92±0.O7, hypertension:2.98 ±0. 14, class 1:2.92 ±0.07, class 2:2.99±0.10, class 3:3.12±0. 17), alpha-microglobulin, uric albumin were elevated in essential hypertension group and in class 2 or 3 hypertension subgroups compared with normal group.Serum Cys C had stronger positive correlation with serum creatinine (r=0.8 ,P〈0.001 ) than alpha-microglobulin, alphamicroglobulin, uric albumin. Conclusion Serum Cystatin C may be a marker of early renal injury in patients with essential hypertension.
