摘要
目的探讨葛根素合并纳洛酮治疗脑震荡的效果及作用机制。方法将121例脑震荡患者随机分成纳洛酮(NLX)组(n=40),NLX+葛根素组(n=41)和对照组(n=40),采用经颅三维多谱勒(3D-TCD)动态检测脑血流变化,结合临床近、远期疗效综合评定葛根素和纳洛酮治疗脑震荡的疗效。结果纳洛酮组近、远期疗效分别为80%和85%,对照组为62.5%和72.5%(P<0.05)。NLX+葛根素组在解除脑血管痉挛,改善脑血流和远期疗效(92.6%)明显优于纳洛酮组。结论纳洛酮可以阻断内源性阿片肽对中枢神经系统的抑制作用,改善缺血时神经细胞供氧,如果与葛根素联合用药有助于缓解脑血管痉挛降低脑血管阻力,增加脑血管流量,改善微循环,从而提高脑震荡的疗效,降低脑震荡后遗症的发生率。
Objective To observe the clinical effect and mechanism of puerarin and naloxone in treatment for concussion. Methotis 121 patients with concussion were randomly divided into the naioxone(NLX)group(n=40), NLX+ puerarin group(n=41)and control group(n=40). Cerebral blood flow were measured dynamically by color three-dimrnsional transcranial Doppler(3D-TCD). Clinical effects of puerarin and naloxone was evaluated synthetieaUy according to short-dated and long-dated results,Results The short-dated and long-dated effectual rate was 80% and 85% respectively in naioxone group,62.5% and 72.5% in control group(P 〈%0. 05). In comparison with NLX group, NLX+ puerarin group was significantly better in long- dated effect(02.6 %) and releasing cerebral vessel spasm. Conclusion Naloxone can obstruet from the depressing of endogenous opioid peptides to central neural system and the neurocyte oxygen supplying improve. Using NLX and puerarin combinativly is conductive to release cerebra vessel spasm and increase cerebral blood flow and improve cerebral mierocirculation,thus the clinical effect improve and concussion sequela decrease.
