13例非小细胞肺癌肋骨转移的外科治疗

Clinical Analysis of Surgical Treatment on 13 Cases of Non-small Cell Lung Cancer with Rib Metastasis

目的:探讨13例非小细胞肺癌骨转移患者的外科治疗特点和预后因素。方法:对1998年7月～2003年7月行手术治疗的非小细胞肺癌骨转移患者13例进行回顾性分析。结果:全部13例患者,生存时间4～38个月,中位生存时间14个月,1年生存率为54%。单发肋骨转移患者的生存期明显长于多发肋骨转移和肋骨外转移者(P=0.015)。肿瘤的病理类型、原发肿瘤(T)和淋巴结转移情况(N)与生存期并无关系。结论:对于①单发肋骨转移②预计原发病灶和纵隔淋巴结能彻底清除③身体条件能耐受手术的患者建议进行积极的外科治疗。

Objective： To describe the relationship between survival rate and tumor characteristics in patients with non-small cell lung cancer （NSCLC） with rib metastasis. Methods： Data from 13 NSCLC patients in our hospital with osseous metastasis were retrospectively reviewed. Results： The median survival of the cases was 14 months （4-38 months） while the 1-year survival rate was 54%. The survival time of the patients with solitary metastasis of the ribs was significantly longer compared to patients with muhiple costal metastasis and metastasis at other sites. No correlation was found between the survival time and tumor histology, primary tumor volume or lymph node involvement. Conclusion： It is suggested that NSCLC patients with solitary rib metastasis that are physically eligible be treated with resection of the primary tumor and lymph nodes.
