摘要
目的探讨慢性阻塞性肺疾病(COPD)患者的血液凝血系统功能的改变。方法采集COPD急性加重期患者82例、稳定期患者25例和20例正常对照者的外周静脉血,测定D-二聚体、纤维蛋白原(Fb)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血时间(TT)。结果COPD急性加重期组与稳定期组比较。D-二聚体、Fb、PT、APTT、TT间差异无显著性。较正常对照组比较,D-二聚体和Fb水平升高,差异有显著性。COPD重度患者与轻中度患者间比较,D-二聚体和Fb水平差异有显著性(P〈0.01)。结论COPD急性加重期和稳定期患者,凝血系统加强,抗凝系统减弱,存在血栓前状态。
Objective To study the function changes of coagulation system in patients with chronic obstructive pulmonary disease ( COPD ). Methods The peripheral vein blood samples were taken from 82 COPD cases at acute worsening period,25 stable cases and 20 normal controls for the determination of D-dimer, Fibrinogen ( Fb ), Prothrombin time(PT) ,Activated partial thromboplastin time(APTT) and thrombin time(TT). Results D-dimer,Fb, PT,APTT and Tr were not significantly different between acute worsening COPD patients and stable patients but D- dimer and Fb level were higher in acute worsening COPD patients than normal controls. D-dimer and Fb level were significanfly different between severe COPD patients and slight and mediate COPD patients ( P 〈 0.01 ). Conclusion Coagulation function is strengthened and anti-coagulation system is weakened in acute worsening and stable COPD patients,in whom prethrombolic state exists.
