脾切除后持续发热病因及治疗方法

Study on the causes and therapeutic method of continuous fever after splenectomy

目的:探讨脾切除术后持续发热的原因及治疗方法。方法:收集10年间行脾切除术的临床资料,对其中26例术后持续发热两周以上的发热原因,治疗进行分析。结果:26例中有23例发热是由各种并发症引起,其中胸腔,膈下积液13例,门脾静脉血栓5例,各种感染并发症8例,不明原因发热3例,持续发热与手术方式无关(P>0.05),与原发疾病有关(P<0.05)。结论:脾切除术后持续发热主要由各种并发症引起,有效控制感染,及时处理并发症能有效减少术后持续发热。

Objective：To investigate the causes and therapeutic method of continuous ferer after splenectomy.Methosd,The clinical data of spleneetomy in recent 10 years were coUected,in which 26 patients with postoperative continuous fever of 2 weeks or more were analysed retrospectively,in order to study its causes and therapeutic method.Results：Of 26 patients with postoperative continuous ferer,23 were caused by various complications,in which the most common complication was the hydrops of subdiaphragmatic space and thoracic cavity in 13 cases,splenoportal thrombosis in 5 cases,various infection in 8 cases,splenoportal thrombosis in 5 cases,various infection in 8 cases and unknown origin in 3 cases.The postoperative continuous fever was unrelated to the operative procedures （P〉0.05）,but had a correlation with primary disease（P〉0.05）.Condusion：The continuous fever after splenectomy was mainly caused by various complications. Controlling the infection effectively and treating the complications in time can effectively decrease the postoperative continuous fever.
