血液透析患者CHr与CRP及铁参数的相关性研究 被引量:1

摘要 目的:研究血液透析者网织红细胞血红蛋白(CHr)、CRP及铁参数如血清铁蛋白、转铁蛋白饱和度(TS)之间的相关性。方法:对肾衰患者同时补充铁剂和红细胞生成素(EPO)后,测定其CRP、CHr、低色素红细胞百分比(%Hypo)、血清铁蛋白、TS等参数的测定,将C-RP与CHr、铁蛋白、%Hypo之间及CHr与%Hypo、TS、血清铁蛋白之间进行相关性比较。血清铁蛋白、TS、%Hypo、CHr在高CRP值(≥8mg/L)和低CRP值(<8mg/L)时的两两比较。结果:CRP对数与CHr呈明显的负相关(rho=-0.085,P<0.001),而与铁蛋白呈明显的正相关(rho=0.52,P<0.001);与%Hypo呈正相关(rho=0.23,P<0.001)。CHr与%Hypo呈明显的负相关(rho=-0.31,P<0.001);与TS呈明显的正相关(rho=0.28,P<0.05);与血清铁蛋白无相关性(rho=0.96,P>0.05)。以CRP的均值(8mg/L)为界值分别将参数分为两组(CRP≥8mg/L组和CRP<8mg/L组)进行比较,用两样本的u检验,血清铁蛋白在两组之间差异有显著性(P<0.05),而TS、%Hypo及CHr在两组之间差异无显著性。结论:当炎症存在时,CHr受到严重影响,体内的贮存铁很难达到理想水平。
