人胃液内表皮生长因子放免测定的临床研究

Clinicatl Study on Epidermal Growth Factor Levels Measured By Radioimmunoassay in Human Gastric Juice

为获取正常人胃液内表皮生长因子（ＥＧＦ）的生理浓度，探讨ＥＧＦ在胃、＋二指肠良恶性疾病中可能产生的作用，初探ＥＧＦ与幽门螺杆菌（Ｈｐ）感染间的相互关系，用放免方法测定了不同年龄组正常人、慢性胄炎、消化性溃疡、胃癌患者胃液内ＥＧＦ的含量，并对部分活动性胃炎及活动期溃疡伴Ｈｐ感染的患者给予抗Ｈｐ及抗溃疡治疗，测定其治疗前后ＥＧＦ含量变化。结果表明各年龄组间胃液内ＥＧＦ含量无显著差异，与年龄关系不大；胃癌患者胃液内ＥＧＦ含量明显增高，胃炎及溃疡病患者明显降低，提示ＥＧＦ分泌低下可能与胃良性疾病的发病有一定关系；ＥＧＦ浓度与胃肿瘤发生也有一定关系。随着Ｈｐ清除，组织学胃炎改善，溃疡的意合，ＥＧＦ含量恢复正常。提示Ｈｐ可能影响ＥＧＦ的生成与分泌。

The purpose of the present study was designed to obtain physiologyical levels of EGF in normal human gastric juice and to discuss capable role of EGF on the benignant or malignant diseases gastric and duodenal and to probe into their relation gastric and duodenal and to probe into their relation between EGF and HP. The levels of EGF in gastric juice were measured by radioimmunoassay in 86 normal human, 113 patients with chroinic superficial gastritis (CSG). gastric ulcer (GU) duodenal ulcer (DU) and progressive gastric carcinoma. Anti-ulcer and Anti-HP treatments were taken in a part of the patients with active gastritis and peptic ulcer companied with HP.The defference of the levels of EGF was observed between before and after treatment. The results showed that:(1)The level of gastric juice was respectively,0. 62±0. 15ng/ml,0. 36±0. 14ng/ml,0. 33±0.15ng/ml,0. 24±0. 10ng/ml,1. 51±0. 49ng/ml,in normal human,patients with CSG,GU,DU,and Ca.(2)There were not distinctly differences in the levels of EGF among several different age groups (P>0.05).(3)The levels of EGF were reduced in the patients with CSG, PU and rised in patients with gastric carcinoma;(4)The levels of EGF were gotten normal levels,as a consequence of HP cleared out of from the gastric mucosa,microscopic gastritis improved and ulcer healed.We conclude that①The deficinency of EGF release may be involved the pathogenesis of gastritis and peptic ulcer diseases.②The synthetic and release of EGF may be affected as HP infection in gastric mucosa.
