全直肠系膜切除与双吻合器技术治疗低位直肠癌

Total mesorectal excision and the double stapling technique in anus reservation of low rectal cancer

目的：评价全直肠系膜切除、双吻合器技术在低位直肠癌保肛手术中的应用。方法：回顾性分析162例采用直肠全系膜切除、双吻合器行低位直肠癌前切除术患者的临床资料。结果：术后发生吻合口漏6例（3．7％），吻合口狭窄3例（1．9％），吻合口出血2例（1．2％）。术后随访6个月～7年，局部复发11例（6．79％）。结论：低位直肠癌全直肠系膜切除能降低局部复发，而双吻合器的应用能提高低位直肠癌的保肛率。

Objective To evaluate the values of both total mesorectal excision and the double stapling technique in low rectal cancer. Methods One hundred and sixty-two patients with low rectal cancer received rectomy with total mesorectal excision and the double stapling technique. Results The anastomotic leakage occurred in 6 patients, and anastomotic stricture occurred in 3 cases anastomotic bleed occurred in 2 cases. All patients were followed up from 0.5 to 7 years, local recurrence occurred in 11 patients. Conclusion Total mesorectal excision can decrease the rate of local recurrence in low rectal cancer, and the double stapling technique can increase the probability of anus reservation.
