非胰岛素依赖型糖尿病病人红细胞形态变化及其影响因素的观察

STUDY OF ERYTHROCYTE MORPHOLOGY AND AFFECTING FACTORS IN PATIENTS WITH NON-INSULIN DEPENDENT DIABETES MELLITUS

（１）目的：观察非胰岛素依赖型糖尿病（ＮＩＤＤＭ）病人红细胞形态变化及其影响因素，（２）方法观察６４例ＮＩＤＤＭ病人外周血红细胞扫描电镜形态，同时检测了红细胞膜胆固醇（Ｃｈ），磷脂（ＰＬ），经质过氧化物（ＬＰＯ）含量的变化，并与３９例健康人进行对照，（３）结果ＮＩＤＤＭ病人红细胞膜Ｃｈ，Ｃｈ／ＰＬ和ＬＰＯ含量明显增高，而ＰＬ明显降低，与对照组比较，棘形，嵴形，球形等红细胞。

Objective To study the morphological changes of erythrocyte and its affecting factors in patients with non-in-sulin dependent diabetes mellitus(NIDDM). Method The erythrocyte morphology, the membrane cholesterol(Ch), phospho-lipid(PL) and liperoxide(LPO) were studied in 64 NIDDM patients, and compared with 39 healthy persons. Result The abnormal morphology rate was significantly higher in the patients than in the controls(t = 4. 826, P<0. 001), and there were stomato-cyte, acanthocyte, ridgecyte, spherocyte erythrocyte in peripheral blood. The content of erythrocyte membrane Ch, Ch/PL and LPO were significantly higher, PL was significantly lower in the patients than in the controls(t = 3. 765~4. 829, P<0. 001). The changes of the above indicators in NIDDM patients with angiopathy were more obvious than in patients without angiopathy (t = 4. 126~5. 987, P<0. 001) The morphology abnormal rate was positively correlated to Ch, Ch/PL and LPO(r=0. 461 - 0. 506, P< 0. 001), negatively correlated to PL(r= - 0. 378, P<0. 001). Conclusion The lipid abnormalities of erythrocyte membrane in NIDDM patients are important factors affecting the erythrocyte morphology.
