两种方法治疗特发性膀胱过度活动症的比较

Effect of Sacral Nerve Electrical Stimulation and Tolterodine for Primary Female Overactive Bladder

目的比较电刺激骶神经与托特罗定治疗女性特发性膀胱过度活动症(IOAB)的疗效。方法将40例女性IOAB患者随机分为A,B两组,A组采用经皮穿刺电刺激骶3神经治疗,B组口服托特罗定2mg,1次/d。治疗3个月,比较两组治疗前后排尿日记及尿动力学参数、抑郁与焦虑的心理评分。结果治疗前后排尿日记及尿动力学参数比较,两组差异具有统计学意义(P<0.01)。治疗前后两组抑郁与焦虑的心理评分A组差异具有统计学意义(P<0.01),B组差异无统计学意义(P>0.05)。结论骶神经电刺激与托特罗定均可改善女性IOAB患者的排尿功能障碍,但骶神经电刺激无副作用,改善生活质量较明显。

Objective To compare the curative effect of sacral nerve electrical stimulation and Tolterodine for female overactive bladder. Methods 40 female patients with idiopathic overactive bladder （IOAB） were randomly divided into A and B groups. The patients in group A accepted percutaneous sacral nerve （at S3 ） electrical stimulation test, while the patients in group B were treated with tolterodine 2mg/day. After 3 months, the index of emiction and voiding dynamics of the 2 groups were compared, and the rating of depression and anxiety were evaluated. Results There was significant improvement in variables included the frequency of voiding, volume voided and signs every day and urgent uresis in the both groups. But the rating of depression and anxiety were less improved and had stimulating symptom in the urethra or bladder in group B. Conclusions Single administration of sacral nerve electrical stimulation and tolterodine are both effective for IOAB. But the sacral nerve electrical stimulation is more reliable for its less side effect and successfully improvement to the life of patients.
