摘要
目的:探讨维生素C对Ⅱ型糖尿病(non-insulin dependent diabetes,NIDD)患者血管内皮功能的保护作用。方法:选择新诊断的NIDD患者30例,随机分为2组(各15例),一组采用饮食、运动及胰岛素治疗(胰岛素组),另一组在胰岛素组基础上加用维生素C 0.1,3/d(维生素C组)。治疗6个月。同期,选择14例健康个体作为对照。采用血管外超声法检测肱动脉血流介导的内皮依赖性血管舒展功能(EDD)和硝酸甘油(GNT)介导的内皮非依赖性血管舒展功能。结果:与对照组相比,NIDD患者治疗前空腹血糖(FPG)、2 h血糖(2 hPG)、胆固醇(TC)、三酯酰甘油(TG)明显增高(P<0.05),且低密度脂蛋白胆固醇(LDL-C)亦明显增高(P<0.05),而高密度脂蛋白胆固醇(HDL-C)明显降低(P<0.05),同时伴随EDD明显降低(P<0.05)。不论在维生素C组还是在胰岛素组,与治疗前比较,治疗后HbA1c、FPG、2 hPG、TC、TG、LDL-C水平明显降低(P<0.05),同时伴随EDD明显改善(P<0.01),维生素C组治疗后EDD明显高于胰岛素组治疗后(P<0.05)。结论:维生素C明显改善NIDD患者血管内皮功能。
Objective :To investigate the protective role of vitamin C on endothelial function in non - insulin dependent diabetes (NIDD). Methods: Thirty newly diagnosed patients with NIDD were randomly divided into 2 groups: One group (insulin group) accepted combined intervention therapy (including diabetic diet, regular aerobic exercise, and insulin ) for six months, and another group (vitamin C group) accepted 0.1 grams of vitamin C orally (three times per day). Fourteen healthy individuals (control group ) were selected. Endothelium- dependent arterial dilation (EDD) and endothelium -independent arterial dilation were measured by using high resolution ultrasound technique before and after intervention therapy. Results: Compared with control group, the FPG,2 hPG ,TC ,TG and LDL - C levels in NIDD were significantly higher before therapy ( P 〈 0. 05 ), and the HDL - C and EDD levels were lower markedly ( P 〈 0. 05 ). After treatment, the concentrations of HbA1c, FPG, 2 hPG,TC,TG,LDL- C in both insulin and vitamin C groups were decreased significantly (P 〈0.05), and the EDD in both insulin and vitamin C groups was improved significantly (P 〈0.01 ). Furthermore, the EDD in vitamin C group was significantly higher than that in insulin group after treatment (P 〈 0. 05). Conclusion: Vitamin C can improve vascular endothelial function in NIDD.
