冠状动脉16层CT成像在评价冠状动脉狭窄中的可靠性及价值

摘要 目的:以常规冠状动脉造影为标准,比较16层螺旋CT冠状动脉血管成像(CTA)在评价冠状动脉狭窄中的可靠性及价值。方法:56例临床怀疑冠心病患者分别行冠状动脉造影和16层CTA,对各个冠脉节段进行比较分析,得出CTA诊断冠状动脉狭窄的敏感度、特异度、阳性预测值、阴性预测值。结果:16层CT诊断轻、中、重度冠状动脉狭窄的敏感度、特异度、阳性预测值、阴性预测值分别为88.89%、98.09%、84.21%、98.72%,94.59%、99.97%、87.50%、99.59%,92.50%、97.52%、75.51%、99.37%。结论:16层螺旋CT可作为诊断冠心病的筛选手段以及冠脉手术术前评价和术后复查的常规方法。 Objective: To compare the value and reliability in the diagnosis of coronary artery stenosis between sixteen slice spiral CT coronary angiography and conventional selective coronary angiography(SCA).Method: 56 cases with suspected coronary heart disease (CHD) were consecutively analyzed using 16 slice spiral CT and SCA. Every segment of the coronary artery was compared. The sensitivity,specificity,positive and negative predictive value were measured. Results: The sensitivity, specificity , positive and negative predictive value to identify mild, medium and severe stenosis with sixteen slice spiral CT coronary angiography were 88.89%、98.09%、84.21% 、98.72%,94.59%、99.97%、87.50%、99.59%,92.50%、97.52%、75.51%、99.37%.Conclusion: The sensitivity,specificity ,positive and negative predictive value of 16-slice spiral CT coronary angiography can meet the needs of clinical requirement. It had great value in the screening for suspected coronary artery disease and for pre- and post- operative examination.
