CXCR4在浸润性乳腺癌组织中的表达及其意义

摘要 目的研究CXCR4在浸润性乳腺癌中的表达,并分析与相关临床病理指标、肿瘤转移和患者生存的关系。方法采用免疫组织化学方法研究CXCR4在126例浸润性乳腺癌组织中的表达情况。分析浸润性乳腺癌中CXCR4表达与肿瘤类型、腋窝淋巴结受累数目、临床分期、肿瘤大小、组织学分级、ER、PR、HER-2、PCNA和p53的相关性和CXCR4表达水平对肿瘤转移和患者生存状况的影响。结果CXCR4在浸润性乳腺癌中的表达与腋窝淋巴结受累数目、临床分期、HER-2表达情况和PCNA表达情况呈正相关,与ER、PR表达情况呈负相关,与肿瘤直径、组织学分级和p53表达情况无明显相关性。淋巴结阳性组浸润性乳腺癌CXCR4阳性表达率为67.1%,淋巴结阴性组浸润性乳腺癌CXCR4阳性表达率为48.9%(P<0.05)。126例浸润性乳腺癌患者中发生远处转移组CXCR4表达水平高于未发生远处转移组(P<0.01)。CXCR4表达水平影响患者生存率,表达高者总生存率较低(P<0.05)。多因素分析显示CXCR4弱阳性组、阳性组、强阳性组死亡相对危险度依次为5.315、7.417、15.868,具有随阳性强度增强而增高的趋势。结论CXCR4表达可作为预测乳腺癌转移和预后的独立因素,并可成为乳腺癌治疗的新靶点。 Objective This study was designed to observe and study the expression of CXCR4 and its association with clini-pathological features, metastasis and survival in human invasive breast cancer. Methods Expression of CXCR4 were determined by IHC (immunohistochemistry) technique. A total of 126 invasive cancer samples were analyed in correlation of levels of CXCR4 expression with tumor type, axillary lymph node status, TNM stage, tumor diameter, histological grade, ER status, PR status, HER-2 status,p53 status, PCNA status, metastasis and survival rate over a median 144 months follow-up period, Results Expression of CXCR4 in invasive breast cancer was related with axillary lymph nodal status,TNM stages, HER-2 status and PCNA status positively,as well as with hormone receptor negatively (both ER and PR) ,but was not related with tumor diameter, histological grade and p53 status. The level of CXCR4 expression showed a significant difference between lymph node-positive group and node-negative group (67, 1% vs, 48.9%, respectively, P 〈 0, 05 ) , The level of CXCR4 expression in distant metastasis group was higher than that in non-far metastasis group ( P 〈 0.01 ) . The overall survival rate in CXCR4 positive groups are lower than that in CXCR4 negative group ( P 〈0.05 ) . The RR( relative risk) in weak,moderate and strong CXCR4 expression group is 5. 315,7. 417,15. 868 respectively by COX proportional hazard regression model. Conclusion CXCR4 could be a marker to predict metastasis and prognosis and be a potential therapeutic target in patients with breast cancer.
