超声引导下凝血酶注射法治疗股动脉假性动脉瘤对血液流变特性的影响 被引量:8

摘要 目的评价超声引导下凝血酶注射法治疗股动脉假性动脉瘤(pseudoaneurysm,PSA)的疗效及对血液流变特性的影响。方法冠状动脉介入诊治术后股动脉PSA患者16例,在超声引导下穿刺PSA瘤腔并注射200IU凝血酶行封闭治疗,所有病例均在治疗后24h和3~5d复查超声,并在注射凝血酶前及注射凝血酶后48h检测有关血液流变参数。结果11例患者在瘤囊内注入200IU凝血酶后瘤腔即刻闭合,5例患者动脉与瘤腔通道血流明显减弱,在超声引起下压迫5~10min闭合,治愈率100%,无并发症发生。所有病例24h及3~5d后复查无复发。注射凝血酶前后血液流变参数无显著差异。结论该法治疗PSA简便、安全、有效,且对机体血液流变特性无明显影响。 Objective To evaluate the clinical utility of ultrasound-guided thrombin injection (UGTI) for femoral pseudoaneurysms and its effect on hemorheologic characteristics.Methods 16 PSA patients after cardiac catheterization were included in the study. Thrombin with a dose of 200IU was directly injected into sac of the PSA. All the patients were restudied by two-dimentional sonography 24 hours and 3-5 days following UGTI,and hemorheologic parameters were analysed before UGTI and 48 hours after UGTI. Results Successful efficacy was visualized immediately in 11 patients,the remaining 5 patients needed ultrasound-guided compression treatmeat of 5-10 minutes. Follow-up at 24 hours and 3-5 days showed no recurrent PSA in any case.The cure rate of UGTI to PSA was 100%, and no complications.No marked difference was noted in the hemorheologic parameters before and after UGTI.Conclusion UGTI is a simple,safe and effective method for the treatment of PSA. No significant changes were found in the hemorheologic oarameters.
