重症监护病房致病菌及其耐药性分析 被引量:2

摘要 目的:了解2002年1月 ̄2005年12月我院重症监护病房常见感染菌的分布及耐药情况,为临床合理用药提供理论依据。方法:用常规方法对重症监护病房病人的各种临床标本进行细菌的培养、分离及鉴定,药物敏感性试验采用Kirby-Bauer法,WHONET5软件进行数据统计。结果:从338份标本中分离出350株致病菌,其中革兰阴性杆菌(G-B)60.8%(212/350),革兰阳性球菌(G+C)34.86%(122/350),真菌4.6%(16/350),;G-B主要有绿脓假单胞菌38.2%(81/212),大肠埃希菌16.5%(35/212),肠杆菌13.2%(28/212),不动杆菌11.3%(24/212)、嗜麦芽窄食单胞菌10.3%(22/212),克雷伯菌9.0%(19/212),其他杆菌1.4%(3/212);G+C主要有金黄色葡萄球菌41.8%(51/122),凝固酶阴性葡萄球菌30.3%(37/122),肠球菌27.8%(34/122)。G-B对亚胺培南(除嗜麦芽窄食单胞菌外)、头孢他啶、头孢匹肟、阿米卡星的敏感率分别为50.6% ̄100.0%,25.0% ̄78.9%、16.7% ̄90.9%和27.3% ̄94.7%,大肠埃希菌、克雷伯菌中产超广谱β-内酰胺酶(ESBLs)发生率分别为45.7%、42.1%;葡萄球菌对万古霉素敏感率为100.0%、肠球菌对万古霉素敏感率为79.4%,除万古霉素、利福平外,其余的抗生素对MRSA、MRSSCN的耐药性明显高于MSSA、MSSCN。耐万古霉素肠球菌20.6%,及高耐庆大霉素的肠球菌(HLAR)55.9%。结论:重症监护病房的临床医生应积极进行细菌学检查,根据病原菌特点及药敏结果合理使用抗生素。
