生长抑素和栀子联合应用治疗重症急性胰腺炎的实验研究

Experimental study of combination of sandostatin with gardenia jasminoides ellis in treatment of severe acute pancreatitis.

目的 探讨生长抑素联合栀子对实验性急性坏死性胰腺炎（ANP）的治疗作用。方法 经胰胆管逆行注射1．5％去氧胆酸钠0．05ml／100g体重建立大鼠ANP模型。实验动物随机分为5组：对照组、ANP组、善宁组、栀子组、联合组（善宁＋栀子），观察各组3d死亡率、血清淀粉酶、NO、内毒素、TNF-α、IL-6的变化，并对胰腺组织进行病理学检查。结果 与对照组比较，ANP组3d死亡率、血清淀粉酶、NO、内毒素、TNF—α、IL-6均明显升高（P〈0．05），光镜下胰腺组织病理损害明显；栀子组、善宁组及联合组上述各检测指标均较ANP组明显改善（P〈0．05），但以联合组降低最明显（P〈0．05）。结论 联合应用善宁和栀子治疗ANP有协同作用，疗效优于单一的西药或中药治疗。

Objective To study the therapeutic effects of combining sandostatin （SS） with gardenia jasminoides ellis （GJE） in treating acute necrotizing pancreatitis （ANP）. Methods ANP models were induced by retrograde injection of 1.5% sodium deoxycholate. The rats were randomly divided into five groups： ANP group, sham group, SS group, GJE group and combination group. Changes of 3 d mortality, amylase, NO, endotoxin, TNF-α and IL-6 were measured, as well as changes of pancreatic histology were examined by light microscopy. Results Compared with sham group, 3 d mortality （83.3%）, amylase （1447.6±53.6）U/dl, NO （107.8±9. 9）μmol/ml, endotoxin （0. 429 ± 0. 128）EU/ml）, TNF-α （182.5 ± 13.9）ng/L, and IL-6 （149. 7 ± 15.3）ng/L in ANP group were significantly higher （P 〈 0.05）, and the damage to pancreatic tissue was more severe as observed by light microscopy. The above-mentioned indexes of GJE group, SS group and combination group were obviously ameliorated in comparison with ANP group （P 〈 0.05）, especially in combination group （P 〈 0.05）. Conclusions The therapeutic effect of combining sandostatin with gardenia jasminoides ellis in. the treatment of ANP is superior to using Western medicine or traditional Chinese medicine alone.
