颈静脉球瘤影像学诊断

Imaging diagnosis of glomus jugulare tumors

目的探讨颈静脉球瘤的影像学表现及其诊断价值。方法8例颈静脉球瘤均经病理证实,均行MRI平扫,其中,6例行MRI增强扫描,MRA和MRV检查各2例,CT平扫2例,分析其影像学表现。结果CT显示肿瘤密度均匀,周围骨质呈侵蚀性破坏;MRI肿瘤呈等T1长T2信号,“盐-胡椒”征为颈静脉球瘤特征性表现;MRA显示肿瘤区畸形血管及供血动脉,MRV显示静脉窦受压变细。结论CT对显示骨性结构改变有明显优势,MRI,MRA和MRV对显示病变特征、诊断和鉴别诊断有明显意义。

Objective To discuss the imaging findings of glomus jugulare tumors and their diagnostic value. Methods Eight patients with pathologically proved glomus jugulare tumors all underwent MRI scans without contrast enhancement, of whom, additiual contrast enhanced scans（ n = 6）, MR angiography（ n = 2）, MR venography（ n = 2）, and CT plain scans（ n = 2） . The imaging findings in all patients were analyzed retrospectively. Results on CT images, the tumoral Lesions presented homogeneous density and invasive destruction of peritumoral bone structures. All the lesions were hypointense on T1 weighted MR images and hyperintense on T2 weighted MR images, ＂saltpepper＂ sign was characteristic appearance of glomus jugulare tumors. MR angiography demonstrated vascular vessel malformation inferolateral to the internal carotid siphon and artery supply. MR venography demonstrated compression and thining of jugulare vein sinus. Conclusion CT has obvious advantage for demonstrating the destructive change of bone structures while MRI, MRA and MRV have greater value in the diagnosis and differential diagnosis of glomus jugulare tumors.
