DR与传统X线骨骼系统摄影对比分析 被引量:4

摘要 目的比较直接数字X线摄影术(DR)与传统X线在骨骼系统不同部位的成像质量。方法选择我院120例患者骨骼系统不同部位DR平片以及同时期常规X线平片共150对,由5位影像学医师分别对每对图像进行一一对照,观察骨皮质、骨松质、骨膜、周围软组织、骨折、潜在的病理损害以及总体印象7项指标,按5个等级对各项指标进行评定并打分。同时由3位骨科医师从诊断角度对不同部位疾病的成像满意度进行临床评价。结果DR对于骨皮质、骨松质、骨膜、周围软组织、骨折、病理损害的成像质量及总体印象明显优于传统X线,统计学意义差异非常显著(P<0.001)。对脊柱及骨盆疾病DR诊断满意度明显高于普通X线(P<0.05),而对于普通四肢病变,二者满意度相当(P>0.05)。结论在临床骨骼系统应用中,DR图像质量大多数情况下明显优于传统X线,但从临床诊断角度来看,脊柱及骨盆成像时应优先选择DR,而对四肢病变则可首选传统X线检查。 Objective To evaluate the image quality and usefulness of direct digital radiography system versus a conventional screen -film system for different position in skeletal system. Methods 150 paired picture in 120 patients were acquired using direct digital radiography system and screen - film system, five radiologists judged each paired image independently with respect to seven items: cortical bone, spongy bone, periosteum, soft tissue rendition, fracture, the visibility of potentially pathological changes and total impression. Three orthopedic physicians evaluate the image satisfaction degree in different position from the point of diagnostic view simultaneously. Results The digital image quality were rated significantly better than the screen -film system with respect to seven items(p 〈 0. 001 ) . The digital diagnosis satisfaction were better than the screen -film system in spine and pelvis(p 〈 0.05) and similar in acrepathy. Conclusion The image quality of the digital radiography system was superior to that of screen - film system in skeletal system. However, from the point of diagnostic view, digital radiography should be chosen first in spine and pelvis and screen - film first in acrepathy.
