摘要 目的:综述近年来血小板平均容积在各种疾病中的变化规律及其在各种疾病诊疗中的价值。资料来源:应用计算机检索PubMed1982-01/2006-08有关血小板平均体积检测及其在各种疾病中的意义、变化规律方面的文章,检索词为“mean platelet volume”,并限定文章语言种类为English。同时计算机检索中国期刊全文数据库2000-01/2006-08相关文章,检索词为“血小板平均容积”,并限定文章语言种类为中文。此外,手工查阅国内近年的相关文献。资料选择:对资料进行初步评审,并查看每篇文献后的引文。纳入标准:文章所述内容应与血小板平均容积在各种疾病中的变化意义、对诊断治疗及预后评估的价值相关。排除标准:重复研究或Meta分析类文章。资料提炼:共收集到103篇相关文献,37篇文献符合纳入标准,排除的66篇文献为内容陈旧或重复。符合纳入标准的37篇文献中,2篇反映血小板平均容积与凝血功能的关系,其余均涉及血小板平均容积在各种疾病中的变化。资料综合:血小板平均容积与血小板的凝血活性呈正相关,在各种疾病中的改变可反映机体凝血活性的变化与血小板代谢紊乱情况,增高代表血栓性疾病的危险因素,下降则说明机体凝血活性减低。结论:血小板平均容积是一个判断血小板功能的参数,观察其变化规律对各种疾病尤其是血栓性疾病的诊疗有一定的临床意义。 OBJECTIVE: To summarize the changing rules of mean platelet volume (MPV) and its application value in the diagnosis and treatment of various kinds diseases in recent years. DATA SOURCES: A computer-based online search of Pubmed was conducted for articles about the determination of MPV and its significance and changing rules in various kinds of diseases published from January 1982 to August 2006 by using the key words of "mean platelet volume', and the language was limited to English. Meanwhile, China Journal Full-Text Database was searched for related Chinese articles published form January 2000 to August 2006 with the same keywords in Chinese. Besides, relevant domestic articles in the post few years were manually searched. STUDY SELECTION: Articles were selected firstly, and the citations were retrieved. Inclusion criteria: Articles about MPV in the diagnosis, treatment, and prognosis evaluation of various kinds disease. Exclusion criteria: Repetitive studies and Meta analytical literatures. DATA EXTRACTION:A total of 103 pertinent literatures were collected, and 37 of them accorded with the inclusion criteria, while 66 articles with old or repeated documents were excluded. Of 37 accorded articles, 2 were about the relationship between MPV and coagulation function of platelet, while others were involved in the changes of MPV in various kinds of diseases. DATA SYNTHESIS:The level of coagulation activity of platelet was positively correlated with MPV, the changes of which could reflect the changes of coagulation activity and metabolism disorder of platelet. A higher mean platelet volume was a risk factor of thrombosis disease and a lower mean platelet volume meant decreased blood coagulation activity. CONCLUSION:MPV is a parameter that can reflect the coagulation function of platelet. StUdy about the changing rules of MPV has certain clinical value in clinical diagnosis and treatment of various kinds disease especially of thrombosis disease.
