急性白血病患者化疗前后血清TSGF与α_1-AG水平

摘要 目的探讨急性白血病(AL)患者血清恶性肿瘤相关物质群(TSGF)及血清α1-酸性糖蛋白(α1-AG)的水平及其临床意义。方法选择51例AL初治患者及11例复发患者。其中急性淋巴细胞白血病(ALL)33例,初治27例,复发6例;急性髓细胞白血病(AML)29例,初治24例,复发5例。用化学显色法检测其血清中TSGF的水平,采用免疫透射比浊法测定其血清中α1-AG的含量,并与正常对照组比较。结果AL初治组及复发组血清TSGF水平均明显高于正常对照组水平(t=10.986、7.850,P<0.001);AL初治组及复发组血清α1-AG水平均明显高于正常对照组水平(t=41.021、23.658,P<0.001);15例完全缓解(CR)患者血清TSGF水平及α1-AG水平均明显下降,与正常对照组相比较均无显著性差异(t=1.321、0.776,P>0.05),与其化疗前血清TSGF及α1-AG水平比较有显著性差异(t=6.223、28.799,P<0.001);ALL难治组血清TSGF及α1-AG水平均明显高于非难治组(t=4.869,16.167,P<0.001);AML难治组血清TSGF及α1-AG水平均高于非难治组水平(t=2.703,16.672,P<0.05,0.001);血清TSGF水平与α1-AG水平呈正相关(r=0.5265,P<0.01)。结论联合检测血清TSGF及α1-AG水平对于监测AL的化疗效果及复发有一定临床价值。 Objective To study serum level changes of tumor supplied group of factors (TSGF)and α1- acid glyeoprotein (α1-AG) in patients with acute leukemia(AL) and their clinical significance. Methods 51 patients with AL before treatment and 11 relapsing patients were selected. Serum levels of TSGF and cq-AG were measured and compared with those of normal controls. Results Serum levels of TSGF in patients with AL before treatment and those in relapsing patients were significantly higher than those in normal controls (t=10.986,7.850,P〈0.001). Serum levels of α1-AG in patients with AL before treatment and those in relapsing patients were significantly higher than those in normal controls(t=41.021,23.658,P〈 0.001);Serum TSGF levels and α1-AG levels of 15 complete remission(CR)patients decreased significantly, the difference between CR group and normal group was not significant (t=1.321,0.776,P〉0.05). Serum TSGF and α1-AG levels of refractory acute lymphocyte leukemia (ALL) patients were significantly higher than those of remission group, and the difference between them was significant (t=4.869,16.167,P〈0.001); Serum TSGF and α1-AG levels of refractory acute myelogenous leukemia(AML) patients were significantly higher than those of remission group, and the difference between them was significant (t=2.703,16.672,P〈 0.05,0.O01);Positive correlation were shown between serum TSGF and α1-AG (r =0.5263,P 〈0.01). Conclusion Serum TSGF and α1-AG can be used for detecting clinical features and evaluating curative effect in patients with AL.
