容量治疗对腹部局部肿胀麻醉抽脂术循环的影响

Effect of volume therapy on haemodynamics during abdominal liposuction

目的探讨负压肿胀法腹部抽脂术中,容量治疗对患者血压心率的影响。方法将50例腹部单纯性脂肪堆积拟行局部肿胀麻醉下负压抽吸术的女性患者分为对照组(Ⅰ组)和试验组(Ⅱ组)各25例,Ⅰ组以5ml/(kg.h)输入林格氏液,Ⅱ组以12ml/(kg.h)输入林格氏液,观察患者在手术过程中血压(SBP/DBP)、心率(HR)的变化。结果两组患者术前SBP/DBP、HR比较差异无显著性意义(P>0.05)。对照组在手术开始前、开始后1小时、手术结束时的(SBP/DBP)、心率(HR)比较差异有显著性意义(P<0.05)。试验组在手术开始前、开始后1小时、手术结束时的SBP/DBP、HR比较差异无显著性意义(P>0.05)。两组在术后1小时和手术结束时SBP/DBP、HR比较差异有显著性意义(P<0.05)。结论采用局部肿胀麻醉下行腹部负压抽吸术给予适量的液体有助于循环稳定,可增加手术的安全性。

Objectives To evaluate the effects of volume therapy on haemodynamic behaviour in patients undergoing abdominal liposuction. Methods Fifty female patients undergoing abdominal liposuction with local anesthesia were randomly assigned to either the trail group （ Group Ⅰ , n = 25 ） or the control group （ Group Ⅱ , n = 25 ） and received different volume of Ringer＇s solution. The blood pressure （BP） and the heart rate （HR） were recorded during the surgery. Results Before surgery, no significant differences in baseline parameters were observed between groups （ P 〉 0. 05 ）. The observed data failed to show significant difference in baseline, at one hour after surgery beginning and the end of surgery in Group Ⅱ （ P 〉 0. 05 ） ; while there was significant difference in Group Ⅰ （ P 〈 0. 05 ）. Postoperative BP and HR were significant different between groups （ P 〈 0. 05 ）. Conclusion Moderate volume therapy is conducive to maintaining a steady haemodynamics during abdominal liposuction under local anesthesia.
