2型糖尿病下肢血管病变患者中瘦素、白介素-6,C反应蛋白的改变 被引量:10

Alterations of serum leptin, interleukin-6,C-reactive protein during lower limb peripheral arterial disease in patients with type 2 diabetes

摘要 目的:研究2型糖尿病(T2DM)下肢血管病变与瘦素(LEP),白细胞介素-6(IL-6),C反应蛋白(CRP)的关系.方法:对T2DM患者125例进行研究,其中病变组65例,糖尿病组60例,对照组50例,对下肢血管进行B超检测,并测定相应生化指标,结合年龄、病程、踝肱指数(ABI)等进行分析.结果:①病变组病程、血清总胆固醇、载脂蛋白B明显升高,与糖尿病(DM)组、对照组比较差异有统计学意义(P<0.01,P<0.05);病变组ABI明显减低,与DM组比较差异有统计学意义(P<0.01).②病变组LEP,CRP明显高于DM组(P<0.01);病变组,DM组IL-6,CRP显著高于对照组(P<0.01).结论:T2DM下肢血管病变患者存在痩素及炎症因子的增高.对于T2DM病程长、血脂紊乱的患者应及早检测ABI,CRP,IL-6,LEP水平,以便早期预防T2DM下肢血管病的发生发展. AIM: To investigate the association of leptin, interleukin-6, C-reactive protein with lower limb peripheral arterial disease (PAD) in patients with type 2 diabetes (T2DM). METHODS: A total of 125 T2DM patients were investigated with B-mode ultrasonography. Serum markers were measured . These clinical data and age, the duration of diabetes and ankle brachial index (ABI) were analyzed and compared among the patients with PAD (n = 65 ), without PAD ( n = 60) and the controls ( n = 50). RESULTS: (1) The duration of diabetes, Tc, ApeB were significantly increased in the patients with PAD compared with the ones without PAD and the normal controls (P 〈 0. 01, P 〈 0.05). The ABI were decreased in the patients with PAD compared with the ones without PAD ( P 〈 0.01 ). (2) In the patients with PAD, the serum levels of CRP, LEP were significantly higher than those of the ones without PAD ( P 〈 0.01 ). In the patients with PAD and without PAD, the serum levels of CRP, IL-6 were significantly higher than those of the normal controls (P 〈 0.01). CONCLUSION: The serum levels of CRP, IL-6, LEP increased in T2DM patients with PAD. Serum levels of ABI, CRP, IL-6, LEP should be measured earlier in T2DM patients who have long term duration and blood lipid disorders so as to prevent the occurrence of PAD.
