贲门癌患者外周血微转移的检测及其临床意义

Detection of micrometastasis in peripheral blood of patients with cardiai carcinoma and its clinical implication

目的探讨Cytokeratin 20（CK20）mRNA是否可以作为贲门癌患者外周血微转移的标志物。方法以巢式逆转录聚合酶链反应（RT-PCR）技术检测58例贲门癌患者术前、术中、术后外周血CK20mRNA的表达情况。结果58例贲门癌患者术前外周血中CK20mRNA阳性率为36．2％。接受切除术的52例患者术前、术中、术后的阳性率分别为30．8％、42．3％、13．5％。术前组与术中组阳性率的差异无统计学意义（P〉0．05），术前组与术后组阳性率的差异有统计学意义（P〈0．05），术中组与术后组阳性率的差异有统计学意义（P〈0．01）。结论CK20mRNA作为贲门癌患者外周血微转移的标志物，具有较高的敏感性和特异性。

Objective To estimate the diagnostic value of CK20mRNA as a mark to detect micrometastasis in peripheral blood of patients with cardial carcinoma. Methods CK20mRNA in peripheral blood of 58 patients with cardial carcinoma was dctected before,during and after resection by using nested RT-PCR technique. Results The positive rate of CK20mRNA in peripheral blood of the 58 patients was 36.2 %. 52 patients had undergone resection, the positive rates of CK20mRNA before,during and after resection were 30.8 % ,42.3 % and 13.5 % respectively. The positive rate of CK20mRNA in peripheral blood of the before resection group was as high as the rate of the during resection group （P 〉 0.05）, the positive rate of the before resection group was lower than the after resection group（ P〈0.05）, the positive rates we.re significantly different between the during resection group and the after resection group（P〈 0.01）. Conclusion As a mark,CK20mRNA has a high sensibitity and specificity to detect micrometastasis in peripheral blood of patients with cardial carcinoma by nested RT-PCR.
