尼莫地平对脑出血后继发性脑损害保护作用临床研究 被引量:33

The clinical analysis of protect effect of nimodipine on brain damage after cerebral hemorrhage

摘要 目的探讨尼莫地平对脑出血后继发性脑损害的保护作用机理。方法60例脑出血患者随机分为尼莫地平组与常规治疗组,所有患者均分别在治疗前后观察脑水肿、临床神经功能缺损评分、Barthel指数评分、临床疗效及日常生活依赖程度的变化。结果尼莫地平组和常规治疗组治疗后水肿带均明显缩小,尼莫地平组缩小更为明显(P<0.01);治疗后尼莫地平组临床神经功能缺损评分较常规治疗组明显降低(P<0.01);治疗后尼莫地平组较常规治疗组BI评分明显增加,日常生活活动依赖程度明显减低(P<0.01);治疗后尼莫地平组临床疗效总有效率(73%)显著高于常规治疗组(40%)(P<0.05);治疗后尼莫地平组较常规治疗组日常生活活动依赖程度明显减低(P<0.05)。结论尼莫地平治疗脑出血有确切疗效,可减轻脑水肿,保护神经细胞,可减轻患者神经功能缺损程度,提高患者生活质量。 Objective To explore the mechanism of protect effect of nimodipine on succeed brain damage after cerebral hemorrhage. Methods 60 cases of hemorrhage were randomly divided into nimodipine treatment group and normal treatment group. The edema area volumes, the clinical neurological functional deficit scores and the Barthel index scores of all the patients were observed before and after treatment. Results The edema area volumes after treatment reduced apparently in both groups and the decreased amounts of the edema area volumes in nimodipine treatment group was much higher than that in normal treatment group(P〈0. 01). The decrease of the clinical neurological functional deficit scores in nimodipine treatment group was much higher than that in normal treatment group(P〈0. 01);the increase of BI in nimodipine treatment group was much higher than that in normal treatment group (P〈0.01);the total efficiency in nimodipine treatment group (73%) was much higher than that of normal treatment group (40%) (P〈0. 05) ;the dependent level of every day life in nimodipine treatment group was much lower than normal treatment group(P〈0. 05). Conclusions Nimodipine has a favourable effect on patient with cerebral hemorrhage, It can relieve cerebral edema, improve local cerebral ischemia, protect the neural cells, and improve their living quality.
