癌-睾丸抗原NY-ESO-1在食管鳞状细胞癌中的表达及其意义

摘要 目的检测NY-ESO-1蛋白在食管鳞癌中的表达,分析其与肿瘤临床病理参数的关系以及NY-ESO-1蛋白在晚期食管鳞癌免疫治疗中的意义。方法选取113例中晚期食管鳞癌标本构建组织芯片,应用免疫组化EnVision二步法,检测NY-ESO-1蛋白的表达,SPSS统计软件分析所得数据。结果109例有效标本中,有32例NY-ESO-1阳性(29.4%),阳性部位在细胞浆,10例正常食管黏膜中无一例表达NY-ESO-1蛋白。NY-ESO-1蛋白的表达主要集中在肿瘤细胞分化较好的Ⅰ级和Ⅱ级食管鳞癌,阳性率有显著性差异(P<0.01),与肿瘤的大小,浸润深度及有无淋巴结转移无关(P>0.05)。结论NY-ESO-1是目前已知的免疫原性最强肿瘤抗原,可诱导自发的体液免疫和细胞免疫反应,其在晚期食管鳞癌中的表达提示NY-ESO-1可作为食管癌的免疫治疗的靶点。 Objective To investigate immunohistochemical expression of NY-ESO 1, cancer-testis antigen in advanced esophageal carcinoma and to correlate their expression patterns with clinicopathologic parameters. To provide some data for tumor-specific immunotheraphy.Methods Using tissue microarray and immunohistochemical EnVision techniques to analyze the NY-ESO-1 expression in 113 cases of advanced esophageal squamous cell carcinoma. Statistial analysis was done with SPSS program. Results Expression of NY-ESO-1 was observed in 32 cases of 109 effective samples of tissue microarray. Immunohistochemi staining was only detectable in the cytoplasm. However the expression of NY-ESO- 1 in normal esophageal mucosa was negative. NY-ESO- 1 expression was found more frequently in well differentiated tumors. Statistical significance was found between different pathologic grade. Expression rate of NY-ESO-1 has no relationship, however, with tumor size, tumor infiltration depth and lymph node metastasis. Conclusion NY-ESO- 1, which is regarded as one of the most immunogenic antigens known today inducing spontaneeus humoral and cellular immune responses. It may serve as a target antigen for development of immtmotherapy for advanced esophageal carcinoma.
