大剂量赤芍治疗重症急性胰腺炎疗效观察

Clinical study of high dose red peony root decoction in treating severe acute pancreatitis

[目的]探讨大剂量赤芍治疗早期重症急性胰腺炎(SAP)的临床疗效。[方法]将符合SAP诊断标准的52例患者随机分成2组,对照组24例应用常规西医治疗方法(包括:加强监护、禁食、胃肠减压、生长抑素、抗感染等);治疗组28例在常规治疗的基础上应用大剂量赤芍煎剂,予胃管内注入。观察2组腹痛、发热、恶心呕吐症状消失的时间,血淀粉酶、白细胞、血钙、血糖恢复正常的时间及并发症的发生率。[结果]治疗组腹痛、发热、恶心呕吐症状消失时间及血淀粉酶、白细胞、血糖恢复正常时间均快于对照组(均P<0.05);2组血钙恢复正常时间比较,差异无统计学意义(P>0.05);治疗组并发症的发生率低于对照组(P<0.05)。[结论]早期应用大剂量赤芍治疗SAP,可尽快改善患者的症状和实验室指标,明显减少并发症的发生。

[Objective]To observe the therapeutic effect of red peony root decoction （RPRD） in treating severe acute pancreatitis （SAP）. [Methods]Fifty two patients with SAP were randomly divided into two groups. The control group （n=24） was treated with routine treatment （intensive care, fasting, gastrointestinal decompression, somatostatin, antibiotics）. The treatment group （n= 28） was treated with RPRD and routine treatment. Signs （abdominal pain, fever, nausea and vomiting） and serum biochemical criteria （amylase, white blood cell, blood glucose and blood calcium）, complications and mortality were observed. [Results] The treatment group alleviated signs rapidly, compared with the control group（P〈0.05）. The level of amylase, white blood cell and blood glucose quickly recovered（P〈0.05）. The complication incidence rate in the treatment group was lower than that in the control group（P〈0. 05）. There was no statistic difference as to the recovering time of blood calcium between the treatment group and the control group （P〉0. 05）. [Conclusion ]Treating SAP with RPRD in early stage may quickly improve signs and serum biochemical criteria, and reduce the complication incidence rate.
