PPH加硬注术与外剥内扎硬注术治疗老年人混合痔对比研究

Procedure for prolapsed hemorrhoids and sclerotherapy versus external dissection and internal ligation procedure for aged patients of mixed hemorrhoids

目的对PPH加硬注术与外剥内扎硬注术治疗老年人混合痔手术进行对比研究。方法将120例患者采用随机数字法分为试验组(PPH加硬注术治疗60例)和对照组(外剥内扎硬注术治疗60例),对临床资料进行对比分析。结果试验组60例中痊愈11例,显效32例,有效14例,无效3例;手术时间(16.55±3.66)min;住院时间(5.07±1.25)d;疗程(10.85±2.55)d;术后疼痛(Ⅱ、Ⅲ度)24例、出血(>50ml)3例、尿潴留24例、肛缘水肿2例。对照组60例中痊愈10例,显效27例,有效16例,无效7例;手术时间(45.25±7.97)min;住院时间(8.32±1.55)d;疗程(15.47±1.56)d;术后疼痛(Ⅱ、Ⅲ度)57例、出血(>50ml)6例、尿潴留29例、肛缘水肿11例。两组近期无狭窄和严重感染,远期疗效有待观察。结论PPH加硬注术治疗老年人混合痔是一种有效方法,手术简单、安全,近期疗效确切,手术和住院时间短、术后疼痛轻、出血少、缩短了疗程,有效地减少了诱发术后老年人的合并病,有应用推广价值。

Objective To study the clinical application of the improved procedure for prolapse and hemorroids （PPH） plus sclerotherapy to aged patients with mixed hemorrhoids. Methods Totally 120 cases of mixed hemorrhoids were randomly divided into PPH group and control group receiving external dissection and internal ligation procedure. Results Of 60 patients in PPH group, 11 cases were cured, 32 cases obtained good clinic outcome and 14 cases were relieved, 3 cases were invalid ; The operation time was （16.55 ±3.66） min, the hospital stay was （5.07 ±1.25） d, the treatment period was （10.85 ±2.55） d; The cases suffering from postoperative pain, bleeding, retention of urine and edema were respectively 24, 3, 24 and 2. Of 60 patients in control group, 10 cases were cured, 27 cases obtained good clinic outcome and 16 cases were relieved, 7 cases were invalid; The operation time was （45.25 ± 7.97 ） min, the hospital stay was （8.32 ± 1.55 ） d, the treatment period was （15.47 ± 1.56） d; The cases suffering from postoperative pain, bleeding, retention of urine and edema were 57, 6, 29 and 11. Conclusion The improved PPH plus sclerotherapy for aged patients with mixed hemorrhoids is effectual, simple, safe and of less complications.
