肾上腺恶性肿瘤的诊断与治疗

Prognosis and treatment of malignant tumor of adrenal gland

目的探讨肾上腺恶性肿瘤患者的临床症状、病理特点及诊断治疗。方法回顾性分析6例肾上腺恶性肿瘤的临床资料,5例有相关临床表现,所有患者术前均经影像学检查。结果6例患者均经手术治疗,病理诊断分别为肾上腺皮质癌4例、肾上腺转移性乳突状腺癌1例和肾上腺恶性纤维组织细胞瘤1例,术后进行放疗和化疗,4例已死亡,2例存活至今。结论肾上腺恶性肿瘤术前主要通过影像学诊断,确诊需要术后的病理学检查,手术切除是目前主要的治疗方法。

Objective To investigate the clinical pathological characteristics, prognosis and medication of malignant tumor of adrenal gland in 6 cases. Methods Clinical data of 6 cases of malignant adrenal tumors were retrospectively analysed. Five patients presented with relevant symptoms. All of the patients underwent image examination before operation. Results The patients were treated surgically. The pathologically, 4 cases wee diagnosed as having adrenocortical carcinoma, one case having metastatic mastoadenocareinoma and one having malignant fibrous histiocytoma respectively. Radiotherapy and chemotherapy were performed after operation. Four patients had died and the remaining two cases were still alive. Conclusion Malignant tumor of adrenal gland is rare. Surgical resection is by now the optimal treatment. Diagnosis should be made by histological examination though radiological findings may be helpful before operation.
