长春瑞滨联合顺铂治疗晚期头颈部肿瘤的临床观察

Clinical study of vinorelbine combined with cisplatin in the treatment of patients with advanced head and neck carcinomas

目的:观察长春瑞滨联合顺铂(NP方案)治疗晚期头颈部肿瘤的临床疗效和毒副反应。方法:58例晚期头颈部肿瘤患者,接受NP方案化疗。长春瑞滨(NVB)25mg/m2,快速静滴,第1、8天给药;顺铂(DDP)30mg/m2,静滴,第1～3天给药,21天为1周期。化疗2周期以上,然后按WHO标准评价临床疗效和毒副反应。结果:58例患者均可评价疗效,获得CR7例,PR21例,SD16例,PD14例,有效率为48.3%,中位肿瘤进展时间(TTP)为8.5个月。主要毒副反应为骨髓抑制、恶心呕吐和脱发。结论:长春瑞滨联合顺铂治疗晚期头颈部肿瘤的临床疗效较高,毒副反应轻,值得进一步研究。

Objective：To evaluate the efficacy and toxicity of vinorelbine combined with cisplatin in the treatment of patients with advanced head and neck carcinomas. Methods： Fifty-eight patients with end-stage recurrent head and neck cancer were treated with intravenous infusion of vinorelbine 25mg/m^2on d1 , d8 ,cisplatin 25mg/m^2on d1 - d3. The patients were repeatedly treated every 21 days. The efficacy and toxicity were evaluated by standard of WHO after no less than 2 cycles. Results：All 58 cases were available for objective response,CR 7 cases,PR 21 cases,SD 16 cases and PD 14 cases. The overall response rates and median TTP were 48.3% and 8.5 months. The main toxicities were bone marrow suppression and nausea/vomiting. Conclusion：The combination chemotherapy of vinorelbine combined with cisplatin in the treatment of patients with advanced head and neck carcinomas is effective and the side effects can be tolerated.
