摘要
目的探讨代谢综合征(metabolic syndrome,MS)患者胰岛素抵抗(insulin resistance,IR)与血清白细胞介素18(IL-18)水平的关系。方法MS患者及年龄相匹配的正常对照组各18例。以高胰岛素-正常血糖钳夹技术检测葡萄糖代谢率(metabolized rate of glucose,M),评价IR程度;采用酶联免疫吸附法(ELISA)检测血清IL-18水平,用Pearson相关分析法分析血清IL-18水平与M的相关性。结果MS组的M显著低于对照组[(5.76±1.81)mg/(kg.min)vs(8.39±1.25)mg/(kg.min)],差异有统计学意义(P<0.05);MS组血清IL-18水平显著高于对照组[(168.75±45.39)pg/ml vs(99.12±36.64)pg/ml],差异有统计学意义(P<0.05);Pearson相关分析显示,血清IL-18水平与M呈负相关(r=-0.43,P<0.05)。结论MS患者的血清IL-18水平升高,可能与IR的发生有关。
Objective To investigate the relationship between serum IL-18 levels and insulin resistance (IR) in patients with metabolic syndrome (MS). Methods Eighty MS patients and 18 age-matched normal subjects were enrolled. IR was evaluated by hyperinsulinemic-euglycemic clamp technique and serum IL-18 levels were measured by ELISA. Pearson's correlation analysis was used to investigate the relationship between serum IL-18 levels and IR. Results MS patients demonstrated higher serum IL-18 levels than controls [(168.75±45.39) pg/ml vs (99.12±36.64) pg/ml], and metabolized rate of glucose (M value) derived from hyperinsulinemic-euglycemic clamp test was lower in MS patients than that in controls [(5.76± 1.81) mg/ (kg · min) vs (8.39±1.25) mg/(kg · min), P〈0.05]. Serum IL-18 levels showed a negative correlation with M value (P〈 0.05). Conclusion The degree of IR and serum IL-18 levels in patients with MS were higher than that in normal controls. IL- 18 might be involved in the pathogenesis of IR in MS patients.
