摘要
目的探讨各级子宫颈上皮内瘤样病变(CIN)及早期子宫颈鳞状细胞癌组织中血小板反应素-1(TSP-1)的表达及与微血管密度(MVD)的关系。方法采用免疫组织化学SP法检测在10例正常子宫颈组织、13例CINⅠ、22例CINⅡ~Ⅲ及30例子宫颈癌组织中TSP-1的表达情况及MVD。结果正常子宫颈、CINⅠ、CINⅡ~Ⅲ及子宫颈癌组织中TSP-1表达强度及MVD计数分别为(3.7±1.5)、(3.0±1.4)、(1.9±1.2)、(1.7±1.0)及(6.8±1.9)、(13.0±3.6)、(22.7±4.4)、(31.0±5.3)。TSP-1主要表达在正常子宫颈组织基底层。子宫颈癌和CINⅡ~Ⅲ组织中TSP-1的表达明显低于正常子宫颈和CINⅠ组织(P〈0.01)。正常子宫颈与CINⅠ,CINⅡ~Ⅲ与子宫颈癌组之间TSP-1表达差异无显著性意义(均P〉0.05)。MVD随着子宫颈病变级别升高而增加。子宫颈病变中TSP-1的表达与MVD呈显著负相关(rs=-0.603,P〈0.05)。结论TSP-1低表达可能参与了子宫颈癌血管生成过程,联合检测TSP-1和MVD可能在子宫颈癌的早期筛查、诊断、治疗中具有一定意义。
Objective To study the expression of thrombospondin-1 (TSP-1) and microvessel density (MVD) in cervical intraepithelial neoplasia (CIN) and cervical cancer and to analyze their correlations. Methods Immunohistochemical SP method was employed to detect TSP-1 and MVD in 10 cases of normal cervix, 13 cases of CINⅠ , 22 cases of CIN Ⅱ-CIN Ⅲ and 30 cases of cervical cancer. Results The mean TSP-1 scores and MVD counts for normal cervices, CINⅠ , CIN Ⅱ -Ⅲ and cervical cancer were (3.7±1.5), (3.0±1.4), (1.9±1.2), (1.7±1.0)and (6.8±1.9), (13.0±3.6), (22.7±4.4), (31.0±5.3) respectively. TSP-1 was mainly expressed at the normal cervical basal epithelial cell layer. The expression of TSP-1 in cervical cancer and CIN Ⅱ -Ⅲ group was significantly lower than in normal cervix and CINⅠ group (P〈0. 01). But there was no significant difference in the expression of TSP-1 between normal cervix and CINⅠ , CIN Ⅱ -Ⅲ and cervical cancer (P〉0.05). The MVD counts were gradually increased with the progressive cervical lesions. TSP-1 expression was negatively correlated to the MVD count (rs = -0. 603, P〈0.05). Conclusion The decreased expression of TSP-1 in cervical lesions may be involved in cervical cancer angiogenesis. TSP-1 and MVD may be used as biomarkers in the screening, diagnosis and treatment of cervical cancer.
