摘要
目的评价序列特异性引物-聚合酶链式反应(PCR-SSP)法和流式细胞仪两种方法检测人类白细胞抗原(HLA-B27)对强直性脊柱炎(AS)的临床诊断价值,以找出一种准确有效更加适合临床的检测方法。方法将按照纽约诊断标准划分的病例组和对照组分别采用PCR-SSP法和流式细胞仪法检测HLA-B27,分别比较两种方法的敏感度和特异度,并采用PCR-SSP法对阳性病例的HLA-B27等位基因分型。结果PCR-SSP法的敏感度(94.8%)高于流式细胞仪法(88.6%),差异具有统计学意义(χ2=4.55,P<0.05)。24例HLA-B27阳性患者等位基因分型HLA-B*270401和HLA-B*2705共占87.4%。结论PCR-SSP法检测HLA-B27具有较高的敏感度和特异度,是一种快速、简单、有效的检测方法。存在于AS患者的HLA-B27等位基因主要是HLA-B*270401和HLA-B*2705。
Objective To evaluate the diagnostic value of PCR-SSP assay and flow cytometry in detecting HLA-B27 in patients with ankylosing spondylitis (AS), and to obtain an accurate and effective method to to fit the clinical diagnosis. Methods The people involved were divided into AS group and healthy control group according to the New York Conference Standard. HLA-B27 was detected by PCR-SSP assay and flow cytometry, and its sensitivity and speciality were comparied between the two groups respectivety. Then, HLA-B27 alletes of positive cases was gene typed by PCR-SSP assay. Results The sensitivity of PCR-SSP method (94.8%) was higher than that of flow cytometry (88.6%), and had a significant difference (x^2=4.55,P〈0.05). The percentage of alleles of HLA-B * 270401 and HLA-B * 2705 was 87.4 % in the 24 cases. Conclusion The PCR-SSP method is an accurate and effective detection method for the diagnosis of AS.
