慢性阻塞性肺疾病合并肺间质纤维化的临床研究

Study of clinical characteristics of chronic obstructive pulmonary disease with idiopathic pulmonary interstitial fibrosis

目的探讨慢性阻塞性肺疾病(COPD)合并肺间质纤维化(PIF)的临床特点及意义。方法对本院3年来临床确诊的57例慢性阻塞性肺疾病合并肺间质纤维化病例进行回顾性分析研究。结果慢性阻塞性肺疾病合并肺间质纤维化临床表现介于上述两种疾病之间,其肺功能表现主要为混合性呼吸功能障碍,X线胸片、CT及高分辨CT(HRCT)兼有两种疾病的特点。结论慢性阻塞性肺疾病合并肺间质纤维化具有其独特的临床表现,胸部HRCT能提供重要的临床诊断依据。

Objective To investigate the clinical characteristics of the patients with chronic obstructive pulmonary disease （COPD） and pulmonary interstitial fibrosis （PIF）. Methods Retrospective analysis was performed in 57 cases diagnosed as COPD complicated with PIF. Results The clinical presentations of COPD complicated with PIF were intermediate between the 2 disases, the main pulmonary function presentation was mixed ventilatory dysfunction, chest X-ray,CT and HRCT had common characteristics in the 2 diseases. Conclusion The patients with COPD and PIF have special clinical characteristics. The chest HRCT can provide an important evidence for clinical diagnosis.
