摘要
目的:探讨甲状腺乳头状癌中缺氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)和VEGF-C表达的临床意义。方法:应用免疫组织化学法检测73例甲状腺乳头状癌(Ⅰ组)、32例甲状腺腺瘤(Ⅱ组)和35例结节性甲状腺肿(Ⅲ组)患者中HIF-1α、VEGF和VEGF-C的表达。结果:HIF-1α在Ⅰ、Ⅱ和Ⅲ组中的表达率分别为75.3%、18.8%和14.3%;VEGF分别为72.6%、18.8%和20.0%;VEGF-C分别为65.8%、15.6%和11.4%;Ⅰ组的表达率均明显高于Ⅱ、Ⅲ组(均P<0.01)。甲状腺乳头状癌中有淋巴结转移的患者HIF-1α、VEGF和VEGF-C表达均高于未发生淋巴结转移者(均P<0.05),HIF-1α、VEGF与VEGF-C的表达存在相关性(P<0.05)。结论:甲状腺乳头状癌中HIF-1α、VEGF和VEGF-C的表达明显上调,且VEGF、VEGF-C的表达可能由HIF-1α调节,共同参与淋巴结转移的过程。
Objective:To evaluate clinical significance of HIF-1α, VEGF and VEGF-C expression in papillary thyroid carcinoma. Method: HIF-1α, VEGF and VEGF-C expression were detected by immunohistochemical method in 73 patients of papillary thyroid carcinoma(group Ⅰ), 32 patients of thyroid adenoma(group Ⅱ ) and 35 patients of nodular goiter(group Ⅲ), respectively. Result: The expression of HIF-1α in group Ⅰ (75.3 %) was higher than that in group Ⅱ(18.8%) and in group Ⅲ(14.3%) ( P 〈0.01). The similar results were also observed about VEGF and VEGF-C. The positive rate of VEGF and VEGF-C (72.6 % and 65.8 % ) in group Ⅰ were markedly increased, compared with that in group 11 (18. 8% and 15.6%, P〈0.01) or Ⅲ (20.0% and 11.4%, P 〈0.01) . In carcinoma patients with lymph node metastasis, the higher expressions of HIF-1α, VEGF and VEGF-C were observed as compared with those without metastasis( P 〈0.05 ,respectively). VEGF and VEGF-C expression had a positive correlation with HIF-1α ( P 〈0. 05). Conclusion: In papillary thyroid carcinoma, HIF-1α, VEGF and VEGF-C expression are significantly increased. Furthermore,VEGF and VEGF-C expression may be regulated by HIF-1α, involving in the process of cancer cell spreading to lymph nodes.
