关附甲素对急性心肌缺血五指山小型猪血流动力学的影响 被引量:7

摘要 目的:研究分析关附甲素(Ace)对结扎冠脉造成急性心肌缺血的五指山小型猪血流动力学指标的影响。方法:选用五指山小型猪,在冠脉前降支中上1/3至1/2处予以结扎建立缺血模型,随机分为冠脉结扎关附甲素(A组,10只)、非冠脉结扎关附甲素(B组,9只)、结扎生理盐水(C组,9只)3组,分别经Swan-Ganze导管测定急性缺血(或对照)动物用药前后血流动力学指标。采用配对t检验比较各组内用药前后指标变化,采用调整基线资料的协方差比较指标的变化是否存在组间差异。结果:共收集五指山小型猪有效实验资料28只,其中A组10只,B、C组各9只,性别、月龄、身长、体重等基线资料无组间差异。在急性缺血模型中,与生理盐水比较,Ace仅使心率有所下降,肺动脉收缩压略升高,对心排量、肺动脉楔压等其他血流动力学指标无明显影响。Ace在急性缺血(A、C组)或非缺血模型(B组)中对各项血流动力学指标的影响无组间差异。实验过程中,应用Ace可适当减慢心率,未见到不利的血流动力学作用。结论:初步动物实验结果证实,在急性缺血模型中,应用Ace在血流动力学方面安全性较好。 AIM: To analyze the hemodynamic effects of acehytisine (Ace) on the acute coronary ischemic models of Wuzhishan (WZS) micropig. METHODS: Adult WZS micropigs were randomized into group A (coronary ligation with Ace infusion), group B (Ace infusion without coronary ligation), and group C (coronary ligation and saline infusion). Surface ECGs and arterial pressure were monitored during the experiments. Hemodynamic data was obtained through Swan-Ganze catheterization placed fluoroscopically at ischemic baseline and after Ace (or saline) infusion. SPSSll.5 statistic software package was used for data analysis. Paired t test was used to compare the data before and after agent administration between each group. Adjusting baseline covariance was used for group comparison. RESULTS: A total of 28 micropigs (10 for group A, 9 for each group B and C) were selected without significant differences of baseline data. In the acute cardiac ischemic models, Ace only lowered the heart rate, and slightly increased the pulmonary pressure. In the Ace infusion models, ischemia had no impact on the hemodynamics effects. No adverse effects on hemodynamics were found with Ace in ischemic cardiac models except reducing heart rate mildly. Plasma drug concentrations of Ace were stable during the experiment, and there were no difference in group A and B at any time. CONCLUSION: This preliminary study suggests that Ace has no obvious adverse effects on hemodynamics in ischemic cardiac models.
