摘要
目的:探讨颈前路空心螺丝钉内固定治疗成人Ⅱ型和Ⅲ型新鲜齿状突骨折安全性、效果和最佳手术治疗方案。方法:从2001年8月～2005年10月,应用经颈前路空心螺丝钉内固定治疗成人齿状突骨折6例。其中Ⅱ型5例,浅Ⅲ型1例。全部采用单枚空心螺钉固定,术前、术后、末次随访时进行体格检查,拍摄上颈椎正侧位及张口位X线片和CT三维成像检查。结果:随访时间4个月～2年6个月,平均11个月,6例全部获得随访。骨折复位满意,骨折全部愈合率,1例遗留颈部僵硬感,但临床检查无活动受限。均未出现并发症。结论:颈前路空心螺丝钉内固定治疗成人Ⅱ型和部分Ⅲ型齿状突新鲜骨折并发症少,骨折愈合率高,是治疗该类型骨折的最佳选择;正确选择手术适应证、充分的术前准备及复位和精细的手术操作以及术后适当的外固定是治疗成功的关键。
objective：To study the safety and efficacy of odontoid fracture of type Ⅱ and type Ⅲ with anterior screw fixation system. Method： from August of 2001 to October of 2005, six cases of odontoid fracture, five cases of type Ⅱ , one case of type Ⅲ, were treated with anterior screw fixation under the monitor of X-way. Result： All the patients were follow-up with average eleven months. Healing was got in all cases, without any complication. Conclusion： the anterior cervical screw fixation is the best choice to treat the odontoid fracture of type Ⅱ.
