临时性主动脉旁路通道加主动脉置换切除食管癌8例报告

An application of temporary bypass and aorta replacement during the esophagectomy for the patients with advanced esophageal carcinoma—8 case report

目的探讨建立临时主动脉旁路并行主动脉置换对食管癌侵犯主动脉壁手术的可行性。方法于受侵部位主动脉两端插管,建立临时旁路通道,维持循环,切除受侵犯的主动脉,用Gore-Tex人造血管置换后切除食管癌,食管-胃吻合。结果8例患者于术后2周内顺利恢复出院,未发生食管胃吻合口及主动脉壁瘘等严重并发症。结论对于食管癌侵犯主动脉的患者,在有条件的情况下,采用临时性主动脉旁路与主动脉置换术,有助于手术切除,改善患者生活质量,但其远期效果及医疗经济价值有待进一步探讨。

Objective： To approch the feasibility of temporary bypass and neplacement of aorta during esphagectomy for the patients with advanced esophageal carcinoma. Methods： 8 patients with advanced esophageal carcinoma which involved the wall of thoracic aorta underwent esophagectomy successfully by temporary bypass of aorta and the involved thoracic aorta was replaced with artificial vessel of Gore - Tex. Results： The all cases recovered uneventfully within two weeks without complications . Conclusion： Although application of temporary bypass replacement and of aorta during esophagectomy for the patients with advanced esphageal carcinoma, if the technique and equipments available, are in favor of radical resection of the tumor, the distant effects and medical economy are remained to study further.
