高度近视眼并发性白内障后房型人工晶状体植入术的临床研究

Clinical study on intraocular lens implantation for high myopia with complicated cataract

目的探讨高度近视眼并发性白内障的最佳手术方法。方法将200例200眼高度近视眼并发性白内障患者随机分2组:A组100眼,行白内障囊外摘出联合后房型人工晶状体植入术;B组100眼,行白内障超声乳化摘出联合后房型人工晶状体植入术。术后1d、3d、1周、2周、4周、8周、16周观察视力、色觉感知、角膜散光、眼表刺激症状、角膜水肿及皱褶、前房炎症反应、眼压。结果手术时间:A组平均38min;B组平均13min,差别有统计学意义(P<0.05)。术后4周内B组视力、角膜散光、眼表刺激症状明显好于A组,差别有统计学意义(P<0.05),8周后2组视力差别逐渐缩小。术后色觉感知、前房炎症反应和眼压2组无显著统计学意义(P>0.05)。结论白内障超声乳化摘出联合后房型人工晶状体植入术是治疗高度近视眼并发性白内障的最佳手术方法。

Objective To explore the best operative measures for highly myopic patients with complicated cataract. Methods Two hundreds high myopic patients with complicated cataract were divided into two groups.The extracpsular cataract extraction with intraocular lens implantation was performed in group A, and the phacoemulslfication with intraoeulor lens implantation was performed in group B. Visual acuity, colour perception,cornesl astigmatism, eye surface irritation,corneal edema and pucker,anterlor chamber inflammatiOn and intraocular pressure were observed at 1 day,3 days,1 week,2 weeks,4 weeks,8 weeks,16 weeks after operatiOn. The follow-up time was form 1 day to 16 weeks. Results Average operative time in group A and group B was 38 minutes and 13 minutes respectively, the difference was significant in statistics （P 〈0.05） .Visual acuity,cornesl astigmatism,eye surface irritatiOn in patient in group B were better than that in group A at 4 weeks after operation, the difference was significant in statistics（P 〈0.05 ）. The difference in visual acuity of two groups was going down after 8 weeks.The difference in clout perceptiOn,anterior chamber inflourmation,intraclor pressure was not significant （P 〉 0. 05 ）. Conclusion Phacoemuslfication with intraocular lens implantatiOn is the best measures for high myopic patients with complicated cataract. [ Rec Adv Ophthalmol 2007 ;27（3） ：215-217]
