胃肠道间质瘤的临床病理观察及免疫组化分析

摘要 目的探讨胃肠道间质瘤的临床病理及免疫组化特点。方法对37例胃肠道间叶来源的肿瘤进行回顾性分析,观察其临床病理特点,并应用CD117、CD34、Actin、S-100四种抗体对肿瘤进行免疫组化标记。结果原诊为胃肠道间质瘤的20例均表达CD117,阳性率100%;原诊为平滑肌瘤及平滑肌肉瘤的17例表达CD117,阳性率94.5%,其中2例只表达Actin。37例肿瘤中表达CD34阳性率72%、Actin阳性率56%、S-100阳性率10%。结论在37例肿瘤中,35例为胃肠道间质瘤,占94.6%。胃肠道间质瘤是胃肠道间叶肿瘤中最常见的肿瘤;CD117、CD34阳性表达是诊断胃肠道间质瘤最可靠的辅助指标。 Objective To investigate the clinicopathological and immunohistochemical characteristics of gastrointestinal stromal tumors (GIST). Methods Thirty seven cases of mesenchymal tumors of the gastrointestinal tract and contiguous areas were retrospectively reviewed and checked for immunochemistry markers of CD117, CD34, Actin and S- 100. Results All the20 cases diagnosed as GIST expressed CDll7, while the expression rate of CD117 in 17 cases originally designated as leiomyoma and leimyosarcoma was 94.5 %. Only 2 cases expressed Actin. Conclusion There were 35 GIST in all the 37 cases(94. 6%). GIST is the most common mesenchymal tumors which is originated from gastrointestinal tract and contiguous areas. The expression of CD117 and CD34 in tumor cells are the best helpful index to diagnose GIST.
